Master Tips

Before creating log in credentials for any services, create a new email for the client that they and the case manager will have access to. Use this same email for all portals and applications.

Be sure to save and document ALL usernames and passwords for portals and applications.
1. Are you Currently Working with Any Services or Programs?
If yes: Please make an appointment with case manager to discuss next steps.
If no: Proceed through without restriction to question 2, “Do you have food stamps?”
2. Do you have Food Stamps?
If yes: Continue to question 3, “Do you have any income?”
If no: Access the SNAP Meal Assistance page to get more information and apply
3. Do you have any Income?
If yes: Proceed to question 4, “Do you have SSI/SSDI?”
If no: Click on the link for more information and to sign up for Income Assistance
4. Do you have SSI/SSDI?
If yes: Proceed to question 5, “Do you have a part-time job?”
If no: Follow these links to sign up for EAEDC as stop-gap and then sign up for SSI and SSDI (only if you have a documented disability!)
Helpful Tips:

Discuss your eligibility for EAEDC with your case manager – You cannot have any other income to get this cash assistance (may only be subject to exception if you work significantly reduced hours per week).

For SSDI – Client may be denied their first time or even multiple times. If they are truly disabled, be persistent and continue to apply. Make sure the client’s disability is long-term: experienced for more than 6 months and unable to be cured within the next few months.

For SSI – The best application method is calling the local Waltham Social Security office at (800) 772 – 1213 to make an appointment. Please ask the case manager for assistance before continuing.
5. Are you currently Employed?
If yes: Proceed to question 7, “Does your job provide you with health insurance?”
If no: Proceed to question 6, “Do you qualify for unemployement?”
6. Do you Qualify for Unemployment?
To be eligible for Unemployment Insurance (UI) benefits, you must:

Have earned at least:

$6,300 (rounded down to nearest hundred dollars) during the last 4 completed calendar quarters, and

30 times the weekly benefit amount you would be eligible to collect

Be legally authorized to work in the U.S.

Be unemployed, or working significantly reduced hours, through no fault of yours

Be able and willing to begin suitable work without delay when offered
To qualify for benefits, each week, you must:

Be physically able to work

Be available to work

Be actively looking for work (3 work search attempts)
Helpful Tip: If you’re unsure if you can prove that you meet the “actively looking for work” requirement, you can ask your case manager to write a verification letter that you are actively searching for work.
7. Does your Job Provide you with Health Insurance?
If yes: Proceed to question 9, “Do you have a primary care physician?”
If no: Proceed to question 8, “Do you have MassHealth or Medicare?”

If yes: Proceed to question 9, “Do you have a primary care physician?”
If no: Access Medical Programs page to determine eligibility and proceed to sign up.
9. Do you have a primary care physician?
If Yes: Proceed to question 10, “Do you have any urgent medical needs that require immediate attention?”.
If No: Ask case manager for a referral during your appointment and/or view the local resources below:

If you qualify for Local or Federal Section 8 Housing Vouchers, please ask your case manager for a letter of homelessness (supporting documentation that is needed for the application to be approved) during your meeting.

For case managers: If a client is eligible for Local and Federal Section 8 Housing Vouchers, please physically mail both applications to the Waltham Housing Authority at 110 Pond Street, Waltham, Massachusetts, 02451
